CONNECTING TO SUPPORT
Linkage to Care connects individuals with essential services through peers who understand their situation. Get the best support from someone who has been there.

WHAT IS A PEER?
Peers are the outreach specialists. They have experienced many, if not all, of the situations that Linkage to Care aims to assist with including past incarceration, homelessness, and addiction. They are familiar with the barriers that keep people from the help they need and are in a unique situation to be able to provide help and empathy.

Hello, my name is Chris Hayes. As a Peer Support Specialist – meaning someone with lived experience who has had success in recovery – my job is to support you on your recovery journey, helping you achieve success however you define it. I’ve been there. I get it. I’m just here to help.

My name is Sherri, and I’m ready to help you overcome your challenges and live the life you deserve. I know firsthand just how tough recovery can be…because I’ve been exactly where you are right now. If you are ready to make a change and begin your recovery journey, I’m here to help you. Let’s get better…together.
